Registration Form Template

Registration form to be completed by individuals interested in attending a CERT training on Hawaii Island. See the 2008 CERT Schedule for locations, dates and registration deadlines, and indicate these on the top of the form.

CERT Application Template

CERT trained individuals may choose to form a Team within their neighborhood. This template may be completed by the Team members and submitted to the Hawaii County Resource Center. Once this is received, the program guidelines require that criminal back ground checks be conducted on all members.

Hawaii County Civil Defense Agency

The Civil Defense Agency directs funding for CERT Training and equiptment, as well as managing the Community Emergency Response Teams on Hawai`i Island.
